Participation Waiver and Release
1. What I am taking part in
I will take part in activities such as a jiu-jitsu challenge, a balance challenge on a bosu ball, a beginner mini-class or open rolling. The jiu-jitsu activities involve grappling and physical contact, including holds, takedowns and pressure on the body and joints. The balance challenge involves standing on an unstable surface and carries a risk of falling.
2. Risks
I understand these activities carry a risk of injury, including bruises, sprains, strains, broken bones, joint injuries, concussion and, in rare cases, serious or permanent injury. I take part voluntarily and at my own risk.
3. My health
I have no injury or medical condition that makes it unsafe for me to take part, and I am not under the influence of alcohol or drugs. I will tell a coach before I start if that changes.
4. Rules
I will follow the coaches' instructions and the rules displayed at the arena. I will stop at once when told to stop, and I will say "stop" or tap if I am in pain. I will take off my shoes, jewellery and any hard objects before stepping on the mat. I understand the coach's decision on any challenge and prize is final.
5. Release
To the fullest extent allowed by law, I release Lawan Lawan Bintulu Jiu Jitsu, its coaches, team members and helpers, and the event venue and organisers, from any claim for injury, loss or damage arising from my taking part, except where it is caused by their gross negligence or wilful misconduct.
6. Emergencies
I allow the team to give first aid, to call emergency services for me, and to contact my emergency contact.
7. My details
I allow Lawan Lawan to keep the details on this form for safety and to contact me about this event.
8. Under 18
If the participant is under 18, I am their parent or legal guardian, I am present at the event, I have read this waiver, and I agree to it on their behalf.
